At ABBA Healthcare Services, LLC, our Developmental Disability In-Home Service offers individualized, goal-driven support to help clients thrive in their home and community settings. Services are built around each person’s Individual Support Plan (ISP), with an emphasis on daily skill-building, behavioral guidance, and meaningful engagement. Our trained staff work in close coordination with families, support coordinators, and medical providers to deliver care that promotes independence, safety, and personal growth. With a focus on consistency, compassion, and progress, we provide in-home care that supports both immediate needs and long-term development.

Our services include, but are not limited to:

1. Personal Assistance Services

  • Support with Activities of Daily Living (ADLs):
    Bathing, grooming, dressing, eating, toileting, mobility, and transfers—all delivered respectfully and with attention to comfort and routine.
  • Medication Assistance:
    Provided within the caregiver’s scope and state guidelines, including reminders and appropriate support for administration.
  • Health Monitoring:
    Basic wellness checks such as temperature, pulse, and visual observation for health changes.
  • Meal Preparation & Feeding Support:
    Meal assistance aligned with dietary guidelines, personal preferences, and nutritional needs.
  • Household Tasks:
    Light cleaning, laundry, and household organization to support a safe and orderly environment.

2. Skill Building and Training (Based on ISP)

  • Self-Help Development:
    Guidance in hygiene, dressing, and personal routines to support daily self-care independence.
  • Money Management:
    Practical instruction for budgeting, handling money, and completing small purchases.
  • Routine Management:
    Training in time awareness, task sequencing, and maintaining structured schedules.
  • Travel & Mobility Training:
    Support with public transportation skills, community navigation, and safety awareness.
  • Personal Safety Education:
    Emergency preparedness, home safety practices, and awareness of environmental risks.

3. Behavioral Support Implementation

  • Behavior Plan Implementation:
    Care is guided by formal behavioral strategies and reinforcement methods documented in the individual’s plan.
  • Data Reporting:
    Observations related to behavioral goals are accurately recorded and shared with appropriate professionals.
  • Crisis Management:
    Trained staff respond calmly and appropriately during episodes of distress or behavioral escalation.
  • Positive Behavior Reinforcement:
    Encouraging socially appropriate and adaptive behavior through structured reinforcement.

4. Socialization and Community Engagement (As Authorized)

  • Communication Support:
    Assistance with expressive communication, including verbal, nonverbal, and AAC device usage.
  • Community Involvement:
    Accompaniment to social events, volunteer activities, and approved outings.
  • Peer Interaction:
    Guidance in forming and maintaining social relationships and healthy interpersonal skills.
  • Support for Learning & Work Goals:
    Help with education, vocational tasks, and skills aligned with ISP-defined goals.

5. Health Maintenance Activities (As Guided by DMAS)

  • Healthcare Follow-Up:
    Scheduling and coordination of medical or therapy appointments.
  • Medical Visit Support:
    Attendance and assistance during medical visits if approved in the ISP.
  • Assistive Equipment Help:
    Training and support with mobility devices, medical equipment, or communication tools.

6. Family / Caregiver Training and Support

  • Caregiver Coaching:
    Training family members in care techniques aligned with ISP strategies and best practices.
  • Respite Care (If Authorized):
    Short-term care coverage to provide caregivers with rest and relief.
  • Ongoing Updates:
    Transparent communication regarding progress, challenges, and support needs.
  • Access to Planning Resources:
    Tools, education, and guidance for caregivers navigating developmental disability services.

Service Coordination Activities

  • Collaboration with Support Coordinators:
    Ongoing participation in ISP development, progress reviews, and team meetings.
  • Documentation Responsibilities:
    Accurate and timely reporting of care activities, behavioral observations, incidents, and service delivery in accordance with Medicaid or program guidelines.
